This form
is to be completed by
all players, umpires, coaches, club officials and volunteers who wish
to participate in sanctioned baseball competition in Western Australia.

Players game register form
|
To be
completed by the home
team, this form will ensure a complete and accurate record of games
played by players and can be handed in at the canteen by the home team
scorer immediately after the game.

positions throughout the season.

Registration
form - Junior
|
Used to
register a player for Tee-ball,
Little League, Baseball and Softball. This form contains
important information about each individual player such as allergies,
parent contact details and uniform size.
NB: If a player registers
to play Tee-ball as well as Little League or Baseball, they will be
entitled to an $80.00 discount off their combined fees for both sports.

relates to the use of mouthguards,
touches on the subject of insurance,
medical indemnity in the event of injury and registration fees.

Tee-ball
injury reporting form
|
In
compliance with the Healthy
Club and Sport Safety
Policy, this form is used to record
any injuries during the games or training sessions which is then
handed to the Club Secretary who will take any necessary action. |
Prohibited
person declaration
|
Refers to
the Club's Child
Protection Policy and the ineligibility for certain persons to work
or
seek work in various roles within the Club.

Casual player insurance waiver
|
Club insurance indemnity form for casual
players who are not formal Club members. |
